High Intensity Treatment Teams
The High Intensity Treatment Team provides and/or arranges comprehensive services for adults with a serious mental illness. Services include initial and ongoing assessment, treatment planning, coordinating services, advocating, and monitoring. The team is made up of a Supports Coordinator, Assistant Supports Coordinator, Nurse, Certified Peer Support Specialist and Masters Level Clinician. The team meets with individuals on a consistent basis for ongoing support.
Our mission is to provide strength based treatment and recovery supports to adults living with a serious mental illness. We offer encouragement by recognizing and celebrating individuals’ strengths that can be used to improve their quality of life.
High Intensity Services include:
- Initial and ongoing assessment to identify strengths and recovery needs to build the foundation for treatment planning.
- Linking and Advocacy: Support to identify necessary services and assisting in the removal of obstacles that may prevent access to available and appropriate services.
- Care Coordination related to all supports and services an individual is receiving in the community to assure continuity of care and effectiveness of services.
High Intensity Teams support Adults 18 years or older with a serious mental illness or co-occurring disorder, or 1 or 2 physical health risks/chronic health conditions that necessitate assertive outreach/engagement strategies and frequent interactions with treatment providers to stabilize symptoms and reduce complications of illness. Individuals who are re-entering the community or successfully exiting ACT are provided with encouragement and monitoring during these transitions to less intensive treatment supports.
