More Information

An evidence-based comprehensive treatment approach that provides a set of intensive clinical, medical and psychosocial services by a mobile multidisciplinary team in the community. ACT staff believe that people with a mental illness have the right to live independently in the community and need to be the leader of their recovery process.

  • Over 90% of ACT contacts are in the community.
  • All services are tailored through the person-centered planning process utilizing people’s strengths with a focus on recovery.
  • We promote wellness and build on resiliency through psycho-education, identification of warning signs, development of coping skills, medication management and participation in social/recreational groups.
