Supports Coordination

What is Supports Coordination? It involves a variety of services, including:


ASSESSING:  That means we help you identify your needs, wishes, and desires for the upcoming year.

PLANNING: You will have a meeting with all of the individuals who are important to you and are interested in your success.  Your Support Coordinator will help you to plan out what you want in your Individual Plan of Service (IPOS). This plan (IPOS) is like a road map of what you want to work on over the upcoming year.

COORDINATING:  Your Support Coordinator will help bring together the supports and services you have talked about at your Person Centered Plan and are written in your IPOS.

LINKING: When you have services that are outside of the HealthWest system, the Support Coordinator will help you to connect with those agencies, programs or individuals.

MONITORING: Once you have worked on your IPOS the supports coordinator will check in regularly to see how you are doing and how they can help you to succeed in your goals.

ADVOCATING: Your Support Coordinator is the person who will walk along side of you and support you through the entire IPOS process.
